CP Families First Time Visit Registration Form

Welcome to Center Pointe! We cannot wait to get to know your family.  After filling out the information below, please come see us at the blue guest cart in the main lobby and we will help you and your kids get settled in for a great Sunday morning.

Service Times:
CP Kids (Birth-6th grade): Sunday mornings at 8:15, 10:00 & 11:45am

CP Students (7th-12th grade): Sunday evenings 6-8pm
